What is sciatica.?
Sciatica is a common term that means when pain comes in legs anywhere from buttock or low back to toe, it occurs whenever there is pressure on spinal nerves or spinal cord.
What may be causes of pressure on spinal nerves..??
It may be due to many reasons, like slip disc which is most common, infection, swelling in or over the nerve or cord, trauma etc.
What are the reasons for slip disc..??
Disc (Intervertebral disc) is a soft mass structure which present between two adjacent vertebrae, it acts like shock absorber when we bend forward or backward, run, squatting, climbing & in other physical activities.
So it may slip in all these activities if we start aggressively, heavy weight lifting are also a commonest cause, sudden jerk in any activity.
Is it always should be operating a slip disc..??
No, not at all, even I always say > 90 % patient of slip disc either lumbar or cervical can be treated conservatively.
What are the conservative methods..??
I used generally some medicines & exercises with life modification, if patient doesn’t get any relief then I plan for MRI & then treat accordingly.
How much role of physiotherapy in sciatica..??
Physiotherapy plays a good role in treating sciatica or slip disc or low back ache, but it should be suggested by spine specialist (what exercise what type of modality like, TENS, SWT or Ultrasound massage or anything else).
Is, Pain in leg or thigh starts from Low back or hip always related to Spine..??
No, not at all pain may be generating from low back or hip joint also.
Is, Pain in arm or upper limb always related to cervical spine..??
No, not at all pain may be generating from cervical spine, or shoulder joint also.
Where we should consult about spine problems physician/ neurologist/ neurosurgeon/ orthopaedician / physiotherapist or some other specialist..??
Though you can consult with any one, but it always better to consult with spine surgeon it is better to consult ortho spine surgeon because many times pathology generated from joints & presented like spine pathology in that case only orthopaedician can examine in better way & diagnose, is it from joint or spine.
When we should think about surgery of spine..??
If patient not getting relief with conservative management then there may be chance to operate but always after consult with spine surgeon.
What are common symptoms of sciatica..??
Many times patient comes with Low back pain, this pain may be radiate to lower limbs (thigh/leg/foot or whole limb) , if patient feel numbness (“Sunapan/ Bharipan”) in lower limbs or has problem during walk due to pain, or has feeling something running from buttock to toe, it may be due to slip disc, should be consulted with spine specialist.
Is really posture effect our spine problems..??
Yes definitely posture effect spine, even way of sleeping, standing from lying down or way of go to bed all effects the spine problems.
What is the right way to go to bed or on wake up..??
When we want to sleep or rise from bed always we should be in side turn posture with slightly hip & knee bending position.
What are other modalities of treatment other than surgery if patient doesn’t get relief with conservative management..??
I use some drugs inject locally on special places in spine these injections called “blocks”.
What types of surgeries are available for sciatica..??
Now a day’s open surgeries are replacing by minimal invasive surgeries (in which there is minimal incision, minimal blood loss, minimal complication, early rehabilitation, early job joining, early comfortably daily activities & many more benefits).
I am performing very less minimal invasive surgeries with endoscope in which there is no need for anesthesia (surgery generally perform under Local Anesthesia), no blood loss, 8-10 mm incision, it can be perform in patients who not fit for anesthesia , patient can join daily life activities & job very next day . These surgeries are equally effective as conventional open surgeries.
I am also performing Microdisckectomy which is also a minimal invasive surgery.
What is the best time for surgery if patient get paralyses after an accident..??
As early as possible because after an accident it happens due to compression over spinal cord (A bunch of nerves) which should be released as early as possible, the golden period is first 24 hrs in which there are more chances to recover.
How much time takes in recovery if patient operated immediately after??
It depends how much loss was present before surgery, anyhow it’s very very rare to recover immediately. Mostly it takes from month to years in recovery; many times it doesn’t recover completely.
